Posterior Capsular Opacification (PCO) is a condition that can occur after cataract surgery. Sometimes referred to as a “secondary cataract,” PCO can cause vision to become cloudy or blurred again after an initially successful cataract procedure. While this may sound concerning, it is important to know that PCO is treatable, common, and usually very easy to correct.

What Happens During Cataract Surgery?
During cataract surgery, the cloudy natural lens of the eye is removed and replaced with a clear artificial intraocular lens (IOL). The lens sits inside a thin membrane called the lens capsule, which is like a bag and holds the replacement lens securely in place.
In most cases, the back portion of this capsule is left intact during surgery to support the artificial lens. However, over time, some cells that remain in the capsule can begin to grow and cause the capsule to become cloudy. When this occurs, it is known as Posterior Capsular Opacification.
Symptoms of Posterior Capsular Opacification
PCO can develop months or even years after cataract surgery. The symptoms are often similar to the vision problems caused by cataracts in the first place.
Common symptoms may include:
· Blurred or cloudy vision
· Increased glare, especially from lights at night
· Difficulty reading or focusing on fine detail
· Reduced contrast or faded colours
· Vision that gradually becomes less sharp
Because these symptoms can appear slowly, some patients may initially think their cataract has returned. However, cataracts cannot grow back once the natural lens has been removed.
How Common Is PCO?
Posterior Capsular Opacification is one of the most common complications of cataract surgery. It is estimated that around 20–30% of patients may develop some degree of PCO within a few years after their operation.
Modern surgical techniques and improved lens designs have helped reduce the likelihood of PCO, but it can still occur in some patients as part of the natural healing process.
How Is PCO Treated?
The good news is that treating Posterior Capsular Opacification is quick, painless, and highly effective. The standard treatment is a procedure called YAG laser capsulotomy.
